Use the Form to Get Started
Use this form to start the conversation about holding your event at St. Mary’s on Paca Street. You will be contacted by St. Mary’s staff to finalize details and confirm your booking.
This is NOT a reservation form, but it does collect the information St. Mary’s needs to make your event a smooth and productive one.
Using the Form
You can complete the form in your web browser OR you can save it to your computer and fill it out in free Adobe Reader. At the bottom of the form are two buttons:
- One button is to print the form to your printer, which you can then send via regular mail.
- The other button is to save the form to your computer. After you save the form, you can attach it to an email message and send it to info@stmaryspacast.org.
